SAA Aviation Insight Series on Understanding How Complex Aviation Accidents Happen: Unifying Risk Management, Investigation and Oversight
A total of 130 professionals from the airline and aviation industry attended the SAA Aviation Insight Series Talk on Understanding How Complex Aviation Accidents Happen: Unifying Risk Management, Investigation and Oversight on 25 April 2014. Sharing the session was Prof Patrick Hudson, Professor Emeritus of The Human Factor in Safety, Delft University of Technology, Netherlands. Mr Michael Toft, Senior Investigator, Air Accident Investigation Bureau Singapore moderated the question and answer session after the talk.
Prof Hudson shared on a model based on proactive risk management technique that allows the identification of possible causal pathways to accidents such as potential organisational, regulatory and cultural factors. The session also examines the process of accident investigation and analysis and how causation has to be moved from simply deterministic to capture the inherently probabilistic nature of such causes.
